Do I need a doctor’s referral to receive pelvic floor physical therapy?
No. We are direct access providers, so you don’t need a physician referral to see us. If you did get a referral, bring it to your first appointment.
Do you take my insurance?
We are out of network providers and provide a Superbill at the end of your visit that you can submit to your insurer for direct reimbursement.
What should I do to prepare for my first appointment?
There are a few things we ask all patients to bring with them on their first visit to our office.
- Fill out intake forms online before the appointment
- Please wear comfortable clothing that allows easy access to your problem areas.
What kind of clothing should I wear for appointments?
Wear or bring comfortable shorts and a sports bra.
Why is Revive Physical Therapy unique?
We are dedicated to providing each patient with a customized, holistic experience where a physical therapist, not ancillary staff, will spend 55 to 60 minutes one on one with you.
What does a pelvic floor evaluation or treatment look like?
Our pelvic floor physical therapists take a holistic, full-body approach to caring for our pelvic health patients. We assess how the patient’s pelvic floor is functioning with the rest of the body as they move on a daily basis. Sometimes, this involves internal pelvic floor muscle work or assessment and is done only with the patient’s consent. Most of the treatment can also be done externally if the patient isn’t comfortable with internal work.
What is pelvic physical therapy or women’s health therapy?
It is a treatment to help address pain, weakness, and dysfunction in the pelvic floor muscles. These muscles support the bowel, bladder, and sexual functions.
What type of conditions do you treat?
Pelvic pain, Interstitial Cystitis, Vulvodynia, Painful Sex, Vaginismus, Endometriosis, Pelvic organ prolapse, Pregnancy, and postpartum-related conditions, Incontinence, Low back pain, and more.
What office policies should I be aware of?
If you are unable to make it to your appointment due to illness or another emergency, our clinic requests that you call us at least 48 hours before your scheduled appointment time to let us know. Any missed appointments that have not been rescheduled at least 48 hours in advance will result in a non-refundable charge of $85 dollars.
We also ask that if you have been exposed to the COVID-19 virus, please reschedule your appointment with our front desk so that we can maintain a safe, healthy environment for our patients and employees.
